Food Bank Voucher Request

The King Alfred School are proud to link with local community services, such as food banks.

If you would like to request a Food Bank Voucher from the school, we can provide these during term-time. Please fill out the following form and a designated member of staff will complete the voucher for you:

Please note the questions on this form are requested by the food bank and are not held by the school itself. Once completed, the voucher can be:

• Printed, placed in an envelope, and left at Main Reception for collection

• Printed, placed in an envelope, and sent home with your child

• Emailed to you
